The Parks and Recreation Services Department is dedicated to improving the quality of life for the community by providing exceptional programs, facilities and services. In an effort to provide this level of service we have provided our most used forms for you here. Forms can be dropped off at our main office located at 1100 West Street (next to the Cinedome movie theatre) during regular business hours, Monday - Friday, 8:00am- 5:00pm (closed 12:30pm-1:30pm Mon-Fri, weekends & holidays). We also have a drop box at this location for after hour drop-offs.

As of January 1, 2013 to be compliant with Federal Regulations that are in place to protect our customers' identites & sensitive financial information, credit card payments can only be accepted online or when the credit card is presented at the front counter of our administrative office. Fax-in registration will no longer be accepted. We will continue to accept registration by mail or drop box for check or money order payments only. Class Change/Transfer forms and Request for Withdrawl will still be accepted via fax, 707-257-9532 or email, This e-mail address is being protected from spambots. Recreation Activity Forms

Registration Change/ Transfer Request - Use this form to request a change to a class or activity that you are currently enrolled in. Must be submitted prior to the currently enrolled class beginning.

Class/ Activity Withdrawl Request - Use this form if you wish to withdrawl from a class. Please note that once enrolled into a class or activity registration fees are not refundable unless the class is cancelled or a doctor's note is provided verifying that an individual is not able to participate due to illness or injury. This request must be received prior to the completion of the activity.

Youth Sports Forms

Coaching Application - This form must filled out completely by all individuals that have not coached with Parks and Recreation with in the last 12 months. Please note that all volunteers will be LiveScanned at the Napa Police Department prior to being accepted as a coach.

Coaches Update - This form is for individuals that have coached with Parks and Recreation within the last 12 months and have a Coaching Application on file. Adult Sports Forms

A new roster must be filled out completely at the start of each season. Rosters must be turned in with registration and should be complete with team and manager information as well as player names and signatures.
